Oceanic has received isolated reports of DVT assemblies using a specific batch of DVT plungers extruding.

Testing and analysis indicates that this occurs when the First Stage is only partially depressurized prior to the removal from the cylinder. Back pressure from the instrument console could cause the DVT Plunger to extrude.
